Adding FRACAS Failure Reports |
Failure reports are created as children of ‘equipment functional location’ nodes. This feature allows the user to instantly determine which location an item of equipment was located at when a particular failure occurred (see below).
A large amount of information is often associated with a failure. This is why each failure report within the program consists of five tabs:
- The Failure tab comprises the following failure report fields : unique id, description, reported by, date, operating mode, environment and failure mode.
- Corrective actions may be assigned directly to a failure report or alternatively a cause tree may be constructed for a failure report and corrective actions assigned to each of the causes.
- The Corrective Actions tab contains a list of corrective actions associated with a failure report. The following fields are associated with the corrective action: start date and completion date.
- The Causes & Corrective Actions tab contains a hierarchy of causes associated with a failure report. The following fields are associated with a cause: name and description. Each cause has a list of associated corrective actions. The following fields are associated with the corrective action: start date and completion date.
- The Effects tab contains a list of effects associated with a failure report.
- The Analysis tab comprises the following failure report fields: symptom, problem, conclusions and investigated by.
- The Actions tab comprises the following failure report fields: work order number, approved by, approval date and completion date
